Post Placement Agreement Request

I have been advised that, as of July 1, 1999, I can request that the Agency seek a separate judicial review of my post placement requests (see attached).  A Court will make a determination as to whether the post placement communication we’ve agreed to is in the best interests of the child; if so, the court will enter an order, separate from the adoption decree.  This means that if, for some reason, the adoptive parent(s) did not provide post placement, the Court could be asked to order them to do so.  There can be no change of the adoption itself or of custody of the child as a result of any action to enforce the post placement communication agreement.

_______ (Initial Please) I do not request a separate court order.  It will be  sufficient that the adoptive family has indicated they will provide the post placement communication as we’ve agreed.

_______ (Initial Please) I do request that the agency seek a separate court order.  I understand that I may be requested to testify before the Court in order to help the Court establish that this request is in the best interest of the child.
